Diagnosis and treatment of myelofibrosis: a personal perspective

The differential diagnosis may be challenging and can include other entities which may also express the JAK2 V617F mutation, such as chronic myelomonocytic leukemia and/or refractory anemia with ring sideroblasts. Traditionally a difficult disease to treat with only a small proportion of patients eligible for a curative bone marrow transplant, this field has recently changed radically with the introduction of JAK inihibitors, the first in class being ruxolitinib.
